　症例は46歳，男性。右背部痛を主訴に他院救急外来を受診した。右尿管結石の診断で疼痛コントロールを目的に入院した。腹部単純CTで右尿管U1に径5mmの結石と，右精巣内に石灰化を伴う径2.5cmの腫瘤を認めたため，翌日泌尿器科に紹介された。診察時，右背部痛は軽快していたが，右精巣内に拇指頭大の硬結を触知した。超音波でも内部不均一な腫瘤を認めたため，右精巣腫瘍と診断し，手術目的で本院当科へ紹介された。腫瘍マーカーはすべて陰性であった。画像上転移は認めなかった。右高位精巣摘除術を施行し，病理結果は精巣成熟奇形腫であった。精巣成熟奇形腫は転移や再発の可能性もあるので，術後も厳重な経過観察が必要である。

　In a 46-year-old male with right flank pain,an abdominal CT showed a 5 mm right ureteral stone and 25 mm right intra-testicular mass. The levels of serum human β-chorionic gonadotropin,α-fetoprotein（AFP）and LDH levels were normal. Brain and chest CT revealed no metastasis. Right high orchiectomy was performed. Histopathological diagnosis was mature teratoma without malignant germ cell components. We think the patient should be rigorously followed up,because of potential malignancy of testicular mature teratoma.
