嵌頓尿管結石に対する後腹膜鏡下尿管切石術

Our Experience of Retroperitoneal Laparoscopic Ureterolithotomy

　ESWLによる治療が不可能で，TULによる加療後も破砕片の排石遅延が懸念された，水腎症および萎縮腎を伴った68歳，男性の長期嵌頓右上部尿管結石（14×5mm）に対し，後腹膜鏡下での尿管切石術を施行した。本術式は従来の開腹による尿管切石術と比較し，より低侵襲的であると思われ，ESWLやTULがともに無効であるような尿管結石に対して考慮してもよい方法であると思われた。

We performed retroperitoneoscopic ureterolithotomy on a 68-year-old man with a long impacted upper ureteral stone (14 × 5 mm) associated with hydronephrosis and a contracted kidney. The stone was not treatable by ESWL and there was concern for delayed excretion of the broken stone fragments even after treatment by TUL. This mode of surgery appears to be less invasive than conventional laparotomic ureterolithotomy and is considered worthy of consideration in the treatment of ureteral stones which are refractory to ESWL and TUL.
