結節性硬化症を伴う腎血管筋脂肪腫に合併した腎細胞癌

Renal Angiomyolipoma and Renal Cell Carcinoma Associated with Tuberous Sclerosis:A Case Report

　症例は30歳，男性。1歳時に結節性硬化症と診断された。右背部痛を主訴に受診した。超音波，CT,MRl検査で両側腎に腫瘤を認め，腎血管筋脂肪腫が考えられたが，傍大動脈リンパ節の腫大も認めたため，腎悪性腫瘍の可能性も示唆された。経過観察中に患者は胃潰瘍による吐血で死亡した。剖検後の病理組織診断で，両側腎に腎血管筋脂肪腫と腎細胞癌が同時に存在していた。

The patient was a 30-year-old man whose chief complaintwas right flank pain. Diagnosis of tuberous sclerosis wasmade when he was 1 year old. US demonstrated a largemass in bilateral kidney. Because of para-aortic Iymphnode swelling on CT scan and MRI, the malignant tumorwas suspected. The patient died of hematemesis due togastric ulcer and the autopsy was performed. The patholog-cal diagnosis was bilateral renal angiomyolipomas andbilateral renal cell carcinomas, pleomorphic cell type.
