珊瑚状結石を伴う腎血管筋脂肪腫

A Case of Ruptured Renal Angiomyolipoma with Staghorn Calculus Caused by Minor Trauma

　歩行中の転倒を契機に肉眼的血尿が出現した48歳の女性が腎外傷を疑われ入院した。精査後，右腎の珊瑚状結石，血管筋脂肪腫，水腎症と診断し右腎摘除術を施行した。腎血管筋脂肪腫の自然破裂の報告は多いが外傷による破裂の報告は少なく，珊瑚状結石を合併した腎血管筋脂肪腫の本邦報告例はこれまでにない。

A 48-year-old woman presented with macrohematuria just after falling down on the ground while walking. KUBand IVP showed staghorn calculus and hydronephrosis ofright kidney. Renogram showed hypofunction of rightkidney. CTscan showed right renal tumor which containedlow density area of adipose tissue. Under the diagnosis ofright ruptured renal angiomyolipoma with staghorn calculusand hydronephrosis, right nephrectomy was performed. This is the first reported case of renal angiomyolipoma with staghorn calculus in Japan.
