残尿量測定における携帯型超音波断層装置の臨床的有用性と問題点

The Value and Problems of Portable Ultrasound Scanner for the Determination of Residual Urine Volume

　37例について携帯型超音波断層装置（Bladder Scan^TM　BVI 5000）を用いて残尿量を測定し，カテーテル導尿量と比較した。BVI 5000による測定量とカテーテル導尿による測定量には，よい相関（r²＝0.922）が認められた。他方，11例（27.9％）で30ml以上の差がみられた。一方で，両測定法に差を認める症例もみられた。BVI 5000測定量のほうが少なかった例は肥満，少ない膀胱内尿量などのために，超音波走査範囲内に全膀胱が捉えられていない場合が多かった。また，BVI 5000測定量のほうが多かったのは，肉柱形成やTUR-P後などの膀胱壁の不整を伴っている例であった。小児は本装置の適応ではないと思われた。

The bladder residual urine volume obtained by portableultrasound scanner (Bladder Scan^TM BVI 5000) wascompared with that obtained by the urethral catheterizationin 37 patients. The two values correlated significantly (r=0.922, p＜0.05. However, the difference over 30ml wasseen in 11 patients (27.9%).Obese patients and/or smallbladder urine were underestimated. On the other hand,highly deformed bladders were overestimated. The scan-ner was not suitable for children.
