腎盂腫瘍における経尿道的超音波検査の有用性

Usefulness of Transurethral Sonography for Renal Pelvic Tumor

　主に血管内に用いられる体腔内超音波診断装置を経尿道的腎盂エコー法として応用することにより，4例の腎盂腫瘍患者に対し，腫瘍の局在および進展度診断を試みた。その結果，腎盂腫瘍に対する経尿道的腎盂エコー法の有用性としては，（1）尿管鏡的な操作に比べて侵襲が小さいこと，（2）腫瘍の壁内進展度診断が可能であること，および（3）360度全周像を描出し得るため小さな腫瘍も見逃すことが少ないこと，などが考えられた。一方，経尿道的腎盂エコー法の短所としては，（1）大きな腫瘍を詳細に描出することは困難であり，CTやMRIのほうが有用であること，（2）下腎杯に存在する腫瘍の描出には限界があること，などが考えられた。

An ultrasound diagnostic system mainly used for vascular imaging was applied for transurethral renal pelvic sonography in 4 patients with renal pelvic tumors in order to examine its ability to localize the tumor and determine the depth of tumor invasion. The merits of this technique for the examination of renal pelvic tumors include (1) less invasiveness than ureteroscopic methods, (2) the ability to determine the intramural depth of tumor invasion, and (3) the ability to provide an image of the whole circumference of the renal pelvis, reducing the possibility of missing a small tumor.
