腹腔鏡下副腎摘除術を施行した褐色細胞腫の3例

LAPAROSCOPIC ADRENALECTOMY FOR PHEOCHROMOCYTOMA: REPORT OF THREE CASES

　褐色細胞腫3例に対して腹腔鏡を用いて副腎摘除術を施行した。このうち2例は通常の腹腔鏡下手術にて摘出し，1例は気腹を用いず腹腔鏡補助下にて摘出した。気腹した2例は炭酸ガス気腹により血中カテコールアミンの上昇が認められたが，術中血圧コントロールは可能であった。3症例の術後回復は極めて速やかで，術中，術後を通じ重篤な合併症は認められなかった。

The indication of laparoscopic adrenalectomy for pheo-chromocytoma is controversial. We succesfully perfor-med laparoscopic adrenalectomies (LA) for pheo-chromocytoma in 2 patients and a laparoscopy-assisted adrenalectomy (LAA) without pneumoperitoneum in one patient. All patients had neither myocardial injury nor arteriosclerosis and the blood pressures were preoper-atively controlled by α-1 adrenaline blocker and calcium channel blocker. The blood pressure during the operation was controlled by adenosine triphosphate, nitroglycerin and phentolamine.
