腎細胞癌に対する腎保存手術の治療成績

Organ-preserving Surgery for Renal Cell Carcinoma

　1988年12月から1997年11月までに，腎細胞癌に対して当科で施行した腎保存手術22例を検討した。平均年齢は56±11歳，男性16例，女性6例，患側は右9例，左13例であった。22例中4例は絶対的な適応例で，残りの18例は対側腎が正常例であった。腫瘍径は平均直径28±11.2mmであった。術式は1例が核出術を施行し，残りの21例は腎部分切除術を行った。平均手術時間は135±39分，22例中19例は腎血流を遮断し，平均阻血時間は22±6.1分であった。残りの3例は阻血を行わなかった。平均出血量は224±300mlであり，1例のみ輸血を行った。予後は観察期間平均31か月で，両側腎癌の1例が術後29か月目に癌死したが，残りの21例は癌なしで生存中であった。

From Dec.1988 to Nov.1997, we performed organ-preserv-ing surgery on 22 kidneys with renal cell carcinoma. Of these 22 patients, the indication was imperative in 4 patients (the imperative group) and elective in 18 patients (the elective group). Four patients of imperative group consisted of 3 patients with bilateral renal cell carcinoma (2 synchro-nous and 1 asynchronous), 1 patient with chronic renal failure. In the elective group, all 18 patients had a renal cell carcinoma with normal contralateral kidney.
