ダブルJカテーテル留置3か月後に生じた多発性結石の1例

A Case of Multiple Stone Formation around the Double-J Ureteral Catheter 3 Months after the Placement

　22歳，男性。1997年2月，右尿管結石に対して経尿道的尿管砕石術（TUL）を施行した後に，ダブルJカテーテルを留置した。以後，両腎結石に対して体外衝撃波結石破砕術（ESWL）を計3回施行し，経過観察していた。5月，KUBにてカテーテル周囲の腎，尿管，膀胱に多発性結石を認めた。経尿道的にカテーテル抜去を試みた際にカテーテルが断裂し，尿管および膀胱内にカテーテルが遺残した。内視鏡的に結石を破砕した後，上部遺残カテーテルは経皮的に，下部遺残カテーテルは経尿道的に摘出した。

A 22-year-old male underwent transurethral lithotripsy for right ureteral stones with placement of double-J ureteral catheter in the right ureter. And then, extracorporeal shock wave lithotripsy was performed for bilateral renal stones. 3 months after the placement KUB showed multiple stone formation on the renal pelvis, ureter and bladder around the catheter. When we tried to remove the catheter, it was broken down and left in the ureter and bladder. Upper residual part of the catheter was removed percutaneously and lower part was removed transurethraly after endoscopi-cal lithotripsy was performed.
