感染を伴った腎嚢胞破裂

Spontaneous Rupture of Infected Renal Cyst:A Case Report

　患者は65歳の女性。糖尿病でインスリン療法中に発熱を来し，腹部超音波検査，腹部CTで右腎外側被膜下に液体の貯留を認めた。穿刺，ドレナージ術を行ったところ，黄色透明〜やや混濁した穿刺液が得られ，造影により右腎外側部の病変と右腎上極の嚢胞との交通を認めたので，右腎嚢胞自然破裂と診断した。細菌培養検査ではE．coliを検出した。腎嚢胞破裂の報告は少なく，筆者らが検索し得た限りでは，自験例を含めて本邦では15例であった。

A 65-year-old diabetic woman on insulin treatment was suffered from high fever. Ultrasonography and abdominal CT scan revealed a liquid-like lesion occupying the lateral subcapsular space of the right kidney. Percutaneous punc-ture of this lesion was performed and the fluid was drained. The fluid was yellowish and clear~cloudy, and injection of contrast medium showed communication between the lesion and an upper polar cyst, but no communication with the urinary tract. Culture of the fluid specimen yielded E. coli. The patient underwent drainage for a ruptured renal cyst.
