腎外傷を契機に発見されたベリニ管癌の1例

BELLINI DUCT CARCINOMA FOUND INCIDENTALY AFTER RENAL INJURY:A CASE REPORT

　65歳，男性。交通事故を契機に肉眼的血尿を発症した。腎外傷の診断にて保存的に治療されていたが症状の改善をみないため精査が施行された。X線検査にて腎実質から腎盂に広がる腫瘤性病変が認められた。腎細胞癌あるいは腎盂腫瘍の診断にて腎尿管摘除術が施行された。病理学的にベリニ管癌と診断された。

　腎実質から腎盂に広がる乏血管性腫瘍を認めた場合，ベリニ管癌も念頭に置く必要があるものと思われた。

A sixty-five-year-old man with a chief complaint of gross hematuria occurred immediately after traffic accident was referred to our hospital because of prolonged hematuria. By radiological examinations including MRI, CT and retrograde pyelography, a space occupying lesion, 4×2cm in size, spreaded from renal parenchyma to renal pelvis was recognized. A renal pelvic tumor could not be ruled out therefore, he underwent left nephroureterectomy. Histopathological diagnosis was Bellini duct carcinoma of the left kidney.

In evaluating hypovascular tumor extending from renal parenchyma into renal pelvis Bellini duct carcinoma should be in mind.
