心血管障害者に対する経尿道的電気蒸散術

TRANSURETHRAL ELECTRIC VAPORIZATION OF PROSTATE FOR CARDIOVASCULAR COMPLICATED PATIENTS

　心血管合併症のある前立腺肥大症患者14例に対して経尿道的電気蒸散術を施行した。装置は切除鏡に新しいローラー型電極を装着したものを使用した。対象の年齢は平均67歳で，心合併症としてST異常が4例，不整脈が4例，脚ブロックが3例，心房細動が1例，AVブロックが1例，右室肥大が1例であった。これらの手術時間，術前後の残尿，最大尿流量，症状の変化，副作用を検討した。手術時間は平均53分で，術中の副作用もなかった。カテーテル留置期間は平均4日であった。残尿量，最大尿流量，症状スコアーのいずれも術前に比べ有意に改善した。本手術法による前立腺手術はTURと同様な効果があり，より安全であった。

Fourteen benign prostatic hyperplasia (BPH) patients with cardiovascular complication were treated by trans-urethral electric vaporization of prostate (TUV-P) using vapor troad. Average age of patients was 67, and 4 patients had ST abnormality including angina, 4 had arrhythmia, 3 had bundle branch block, one had atrial fibrillation, one hand AV block and one had left ventricular hyperplasia. In all operations TUV-P was performed under spinal anesthesia. Just before vaporiza-tion, all patients underwent tansrectal and transurethral biopsy of prostate.
