レーザー膀胱砕石術を施行した巨大膀胱結石

Holmium Laser Lithotripsy for a Giant Bladder Stone

　症例は58歳，男性。1978年より脊損にて膀胱瘻管理を施行していた。1997年に入り肉眼的血尿が継続したためKUBを撮影したところ，膀胱内に径9.0×6.0×5.2cm，推定245gの巨大結石を認めた。DIP,CTにて結石はほぼ膀胱全体を占めていたが，両側の水腎症は認めなかった。1997年4月2日，全身麻酔下にホルミウム・ヤグレーザー膀胱砕石術を施行した。レーザーは結石の表面をかするように，接線方向に照射した。手術時間は336分であったが，膀胱粘膜を損傷することなく，結石は大部分が砂状になって流出した．術後KUBでは残石を認めず，術後経過は良好で術後2日で退院可能となった。

A 58-year-old man with percutaneous cystostomy 20 years previously visited our clinic because of macrohematuria. Abdominal plain X-ray films revealed a giant bladder stone measuring 90 × 60 mm in size. Holumium laser lithotripsy was selected as the treatment. Under general anesthesia, a cystoscope was introduced into the bladder from the urethra with video monitoring. Through a 550 μm holmium fiber laser was applied to scratch the surface of the stone, rather than working perpendicular. Over 80% of the stone became small sand-like particles that were evacuated from the cystostomy tube.
