内視鏡操作のみで完全破砕できた巨大膀胱結石

Endoscopic Disintegration of a Giant Bladder Stone with Lithoclast®

　症例は88歳，女性。無症候性肉眼的血尿を主訴に受診した。初診時KUB上65×60×60mmの結石が尿道に嵌頓していた。金属ブジーを用いても結石は動かず，膀胱瘻を造設した。経過観察中に結石は尿道から膀胱へ移動した。粘膜麻酔下に内視鏡的膀胱結石破砕術を施行。経尿道的および経膀胱瘻的内視鏡下にリソクラスト^®を用い破砕し摘出した。手術時間は2時間10分であった。リソクラスト^®を用いた砕石術の報告では本症例は最大のものである。

An 88-year-old woman sent to our hospital because of asymptomatic macrohematuria. An X-ray of the abdomen revealed a massive bladder stone measured 6.5×6.0×6.Ocm in size. The stone, which was impacted to the urethra, could not be moved to the bladder with a metal bougie. As the general condition of the patient was poor for vesicolithotomy, cystostomy formation was performed. Later, the stone migrated to the bladder. Then, under local anesthesia, endoscopic stone disintegration was performed with Lithoclast^®. It took 130 minutes for the operation.
