ムチン産生腎盂腺癌の1例

A CASE REPORT:MUCIN-PRODUCING ADENOCAR-CINOMA OF THE RENAL PELVIS

　症例は69歳，男性。患者は左側腹部痛を主訴に入院となった。腎盂原発のムチン産生腺癌により，膿腎症を来していた。腎尿管全摘を試みるが癒着が強く断念し，腎瘻からBCGの注入を行った。一般に腎盂原発の腺癌は予後不良とされているが，BCG療法のみで約4年間増殖を抑制できた。その後，皮膚浸潤を来したが放射線療法で腫瘍を縮小させることができた。腎盂原発の腺癌でこれらの保存療法に反応のあった稀な1例を経験したので，若干の文献的考察を加え報告する。

A 69-year-old man was admitted with a chief complaint of the left flank pain and high fever. He was diagnosed as pyonephrosis caused by large quantity of mucin secreted from renal pelvic tumor. Histology of the tumor proved to be adenocarcinoma. Total nephroureterectomy was ini-tially intended but was not successful because of severe adhesion. Through nephrostomy tube, BCG was installed and tumor progression was not observed for about 4 years in only BCG infusion from nephrostomy.
