　患者は80歳，女性。肝精査中に右腎盂腫瘤を認め，当科に紹介された。膀胱にも腫瘤を認めた。腎盂膀胱癌の診断の下，右腎尿管全摘除術および膀胱部分切除術を施行した。病理組織検査では印環細胞様のパターンを伴う，細胞内粘液産生の認められる細胞が混在した。全体の構築は移行上皮癌の形態を保持するものであったため，粘液産生移行上皮癌，grade 2と診断した。尿路上皮の潜在的多分化能に基づく異分化形質発現によると考えられた。

A 80 year-old female was referred to us for further evalu-ation of the right kidney. Abdominal CT and cystoscopy showed the tumor of the right renal pelvis and the bladder. Serum CEA and CA 19-9 levels were within normal range. We performed right nephroureterectomy and partial cys-tectomy under the diagnosis of right renal pelvic and blad-der cancer. The tissue specimens revealed transitional cell carcinoma, grade 2 with mucus-producing differentiation. 11 months later she underwent the resection of sigmoid colon for sigmoid colon cancer.
