A 51-year-old man with a 5-year history of chronic haemodialysis due to diabetic nephropathy complained of acute left flank pain without any history of trauma. CT and angiography revealed left renal subcapsular hematoma and nephrectomy was performed. No apparent haemorrhagic origin could be detected macroscopically and histologically. Post operatively, rhabdomyolysis with critical hyperpotassemia and ischemic colitis developed, but improved by conventional haemodialysis and colos-tomy.
