尿閉を呈した無菌性髄膜炎

A CASE OF URINARY RETENTION SECONDARY TO ASEPTIC MENINGITIS

　46歳男性，尿閉，発熱を主訴に来院。髄液検査，血液検査の結果より単純ヘルペスウイルスによる無菌性髄膜炎と診断された。尿道バルーンカテーテル留置後，保存的治療にて経過観察した。尿閉の原因は膀胱内圧測定，他の泌尿器科学的所見より排尿筋・尿道括約筋協調不全の存在が疑われた。治療には塩酸タムスロシンが有効で，第61病日に自排尿可能となった。

A 46-year-old man was admitted to our hospital with complaints of urinary retention and high fever. A spinal fluid and serologic examination showed aseptic meningi-tis due to herpes simplex virus infection. A balloon catheter was inserted and conservative therapy was done. From the results of cystometry and other urologic examination, we suspected that urinary retention was caused by detrusor sphincter dyssynergia. He became free of balloon catheter which had been indwelled for 61 days after the administration of tamsulosin hidrochloride.
