A 18-year old female with right renal bleeding was examined systematically, and no other abnormal lesion was found.

The patient underwent 0.25% silver nitrate instillation of the upper collecting system. 24 hours after treatment the patient had right flank pain and high fever. A plain abdominal film and CT scan revealed radiopaque material precipitated in the right urinary tract. Five months after instillation therapy remarkable atrophy of the kidney was recognized.

Dermatologist suspected allergic reaction to silver nitrate from patch test.
