　18歳の女性が，肉眼的血尿を主訴に受診した。膀胱鏡で右血性尿線を認める以外，各種検査で異常なく，特発性腎出血と診断した。入院の上，0.25％硝酸銀腎盂注入を行ったところ，高熱を認め，KUB,CTで腎盂尿管にX線不透過の沈着物を認めた。腎瘻を造設すると解熱したが，その後出血を繰り返したため，腐食によると思われる血管瘻を，経動脈的塞栓術で止血した。銀注6週目の順行性腎盂造影で，腎盂尿管移行部の狭窄を認め，5か月後の排泄性尿路造影では，右腎の萎縮が著明であった。なお，パッチテストで硝酸銀に対するアレルギーが疑われた。硝酸銀の尿路注入による重篤な副作用は，過去4例の報告がみられるのみである。

A 18-year old female with right renal bleeding was examined systematically, and no other abnormal lesion was found.

The patient underwent 0.25% silver nitrate instillation of the upper collecting system. 24 hours after treatment the patient had right flank pain and high fever. A plain abdominal film and CT scan revealed radiopaque material precipitated in the right urinary tract. Five months after instillation therapy remarkable atrophy of the kidney was recognized.

Dermatologist suspected allergic reaction to silver nitrate from patch test.
