高血圧を合併した幼児片側性水腎症の1例

A CASE OF AN INFANT HYPERTENSION SECONDARY TO UNILATERAL HYDRONEPHROSIS

　症例は1歳6カ月の男児。尿路感染の精査を主訴に受診。諸検査より高血圧を併発した片側性水腎症の診断で，腎盂形成術を施行し血圧の改善しえた症例を報告。血圧の改善の機序は，術前の水・Na負荷およびレニン分泌異常の両者が関与したものと思われる。

The patient, a 18-month-old male child, was referred for the suspicion of urinary infection. Clinical examinations confirmed the diagnosis of unilateral hydronephrosis with hypertension, and the hypertension was successfully treated with pyeloplasty. The mechanism of blood pressure elevation was considered to be related to the impaired renal ability to excrete excess sodium and water, and to elevated plasma renin levels.
