先天性陰茎捻転症に対する修復術

SURGICAL CORRECTION OF CONGENITAL TORSION OF THE PENIS

　陰茎の形態異常，尿線の方向異常を主訴に来院した3歳と1歳の先天性陰茎捻転症に対して修復術を施行した。両症例とも明らかな索はなかったが，亀頭冠状部尿道下裂を有し，陰茎は左方にそれぞれ90度，120度捻転していた。手術は冠状溝に沿って環状切開を入れ，皮膚と皮下組織を陰茎根部までBuck筋膜から剥離して捻転を修復したのち，外尿道口を形成した。自験例のような症例においては，本術式がきわめて有用と思われた。

Two boys (three and one years old) presented with a complaint of urinary spraying to the left. Physical examination revealed penile torsion, minor degree of coronal hypospadias without the patent chordee. A coronal circumcising incision was made and the skin was dissected free from the entire penile shaft. Buck' s fascia was dissected off the tissues all around, then the torsion was naturally corrected. Next, the meataladvancement and granuloplasty (MAGPI) was performed. Finally remaining dorsal skin was used to cover the ventral aspect.
