先天性中部尿管狭窄の1例

A CASE OF CONGENITAL MID-URETERAL STRICTURES ASSOCIATED WITH CONTRALATERAL HYPOPLASTIC KIDNEY

　7歳1ヵ月，女児．主訴は腹痛，腹部膨満感．諸検査にて右形成不全腎，右中部尿管狭窄による先天性水腎水尿管症と診断し，狭窄部切除，尿管縫縮，尿管端々吻合術を施行した．術後吻合部の通過性が悪く，ダブルJ尿管ステント留置さらに吻合部バルーン拡張術を施行した後は経過良好である．組織学的に狭窄部の筋層は十分に保たれており，発生要因として胎生期尿管の部分的閉塞再疎通現象の障害に起因していると考えられた．

A 7-year old girl came to our hospital with chief complaints of abdominal pain and distention. She had left congenital mid-ureteral strictures associated with right hypoplastic kidney, and underwent resection of the stenotic ureteral portion, ureteral tapering and end-to-end anastomosis. Postoperatively, the patient had double J ureteral stent indwelled and balloon dilatation of the ureteral portion because of poor passage through the anastomotic ureteral portion. The course after these managements was uneventful.
