副鼻腔転移で発見された腎細胞癌

RENAL CELL CARCINOMA DETECTED BY METASTASIS TO THE PARANASAL SINUS:A CASE REPORT

　60歳の女性に見られた腎細胞癌の副鼻腔への転移症例を報告した。鼻・副鼻腔領域の転移性腫瘍は非常に稀であるが，内訳として腎細胞癌が最も多く，鼻出血の主訴を特徴とする。また，自験例は本邦12例目と考えられ，過去11例中5例は転移巣が先に指摘されている。したがって鼻・副鼻腔領域の転移性腫瘍に対する今後の治療のうえで、腎が原発であることを念頭に置かなければならない。

A 60-year-old female was transfered to our hospital with the chief complaint of epitaxis. MRI and angiography revealed the tumor in the paranasal sinus and the tumor was reseced under the diagnosis of hemangioma. But pathological examination of the surgical specimen showed clear cell carcinoma. Prompt urological examinations disclosed bilateral renal tumors which were resected. Pathologically, the renal tumors were moderately and well differentiated clear cell carcinoma, which metastasized to the paranasal sinus.
