49歳，女性。肉眼的血尿による尿閉で来院。膀胱鏡で右尿管口よりの強血尿を認め，超音波検査，CT，腎動脈造影を行い，腎動脈瘤を伴った右腎動静脈瘻と診断した。右腎動脈塞栓術後腎機能が異常に悪化し，精査の結果，多発性骨髄腫であった。
A 49-year-old female was referred to our hospital because of urinary retention following massive gross hematuria. Bleeding from the right ureteral orifice was seen by cystoscopy and a diagnosis of right renal arteriovenous fistula with renal aneurysm was made on ECHO, CT scan, and renal angiography. Renal function was deteriorated remarkably following renal angiography and the diagnosis of multiple myeloma was obtained after several examinations.
