重複腎盂尿管に発生した上部尿路上皮腫瘍の2例

TWO CASES OF PRIMARY UPPER UROTHERIAL TUMORS IN THE DUPLICATED URETER

　症例1は70歳，男性。左完全重複腎盂尿管の上位腎盂および上位尿管に発生した移行上皮癌。症例2は76歳，男性。左不完全重複腎盂尿管の上位腎盂および膀胱に移行上皮癌が発生した。重複腎盂尿管と上部尿路上皮腫瘍の合併例の報告は，本邦において自験2例を含め21症例にすぎない。その症例についてみると，不完全重複腎盂尿管に発生した上部尿路上皮腫瘍は，下位尿管ならびに尿管合流部に腫瘍の発生が多いことを認めた。

Case I was a 70-year-old male presented with gross hematuria. A retrograde ureteropyelogram revealed a complete duplication of the left ureter and a complete occlusion of the upper segment ureter at 20 cm from the bladder. However, angiograms indicated the neovascularity from pelvic level to blind ending-like area shown by the retrograde urogram. So under the diagnosis of the pelvico-ureteric tumors with the duplicated ureter, left nephroureterectomy with a bladder cuff resection was performed. The histopathological diagnosis was grade 2 transitional cell carcinoma, stage Ⅱ by Cummings' classification.
