46歳,女性,未婚.排尿困難を主訴として来院.小陰唇の高度の癒着を認めた.陰唇癒着症の診断のもと,癒着部切開術を施行した.本症と低エストロゲン状態との関係が従来より指摘されているが,本症例は閉経前の女性で,月経不順もなく,低エストロゲン状態が発症の誘因とは考えにくく,外陰部の感染,炎症,および長年性交渉を経験していないことが発症の主たる原因と考えられた.
A 46-year-old unmarried woman complaining of difficult urination had severe adhesion of the labia minora with a pinpoint opening. The patient underwent sharp separation under spinal anesthesia. Labial adhesions are thought to be caused by a combination of local inflammation and estrogen deficiency. In this case, however, she had normal regular menses, and so estrogen deficiency had little relation to the cause of the adhesions. Preceding genital infection and inflammation, and no sexual intercourse for 15 years might have been the cause of the labial adhesions in this case.
