閉経後の陰唇癒着症

LABIAL ADHESIONS IN A POSTMENOPAUSAL WOMAN: AGE DISTRIBUTION OF 24 ADULT CASES IN JAPAN

　排尿困難を主訴とした61歳女性の陰唇癒着症の1例を経験した。陰唇癒着部切離形成術後，排尿困難は消失し，最大尿流率は術前の9.2ml／秒から62.2ml／秒に改善した。術後1か月間エストロゲン剤を内服し，1年後の現在まで再癒着はない。本邦成人24例の年齢分布で，陰唇癒着症は閉経後，特に老年期で増加する傾向がみられた。高齢化社会の到来に伴って，陰唇癒着症は女性排尿障害の原因として，より注意すべき疾患になると考えられた。

A 61-year-old postmenopausal woman with a compliant of voiding difficulty, was disclosed to have nearly com-plete fusion of the labia minora, leaving a pin-hole opening near the anterior fourchette. Uroflowmetry revealed an obstructive pattern with a maximum flow rate (MFR) of 9.2 ml/sec. The patient underwent sharp dissection of the fused labia under lumbar anesthesia, after which her MFR improved to 62.2 ml/sec. Postoper-atively she had oral administration of estrogen for one month and was advised perineal hygiene. During one year of follow-up, the recurrence did not occur.
