内反性乳頭腫と移行上皮癌の膀胱内発生の1例

INVERTED PAPILLOMA AND PAPILLARY TRANSITIONAL CELL CARCINOMA OF THE BLADDER IN THE SEPARATE LESION

　症例は39歳，女性で，尿管結石により急性腎盂腎炎を発症した。尿管ステント挿入時に偶然，膀胱鏡にて右側壁に乳頭状で有茎性の腫瘍と三角部に表面平滑で有茎性の腫瘍を認めた。膀胱腫瘍の診断にて経尿道的膀胱腫瘍切除術を施行した。病理組織学的には，右側壁は表在性の移行上皮癌で，三角部の腫瘍は内反性乳頭腫であった。内反性乳頭腫と移行上皮癌との強い関連性があると考えられた。さらに内反性乳頭腫は悪性化の可能性もあり，定期的な経過観察が必要と考えられる。

A 39-year-old women was admitted to our hospital because of acute pyelonephritis with left ureteral stone. Cystoscopic observation incidentally revealed a papillary pedunculated tumor at right lateral wall of bladder and a smooth-surfaced, sessile tumor at the trigone. We perfor-med the transurethral resection of the bladder tumor. The histological examination demonstrated that the tumor at the trigone was the inverted papilloma and the papillary tumor at right lateral wall was transitional cell carcinoma. This case strongly suggested that inverted papilloma was correlated with the tapillary pransitional cell carcinoma.
