経会陰的前立腺生検におけるレボフロキサシン250mg予防内服の臨床的有用性の検討

Examination of the clinical usefulness of Levofloxacin 250 mg preventive oral administration in transperineal prostate biopsy

　経会陰的前立腺生検前の予防的抗菌薬として国内のガイドラインではレボフロキサシン（LVFX）500mg内服を推奨している．東京都保健医療公社東部地域病院泌尿器科では加齢に伴う腎機能低下を考慮して通常量より減量したLVFX 250mg予防内服下にて経会陰的前立腺生検を施行しており，この臨床的有用性について検討した．その結果，調査期間内に前立腺生検を施行した178症例で，感染症を3症例認めたが，それに伴う入院期間延長や再来院した症例はなかった．単施設でのランダム化されていない後ろ向き調査であるが，これまで報告されたことのないLVFX 250mgを経会陰的前立腺生検前に予防内服することの有用性を示唆する結果であった．

Abstract

In the prophylactic antimicrobial treatment before transperineal prostate biopsy, the Japanese guideline recommends oral administration of levofloxacin (LVFX) 500mg. However considering decrease of kidney function with aging, we examined the clinical usefulness of LVFX 250mg preventive oral administration before transperineal prostate biopsy. In 178 cases only three cases developed infection, far there were no cases with extended hospitalization period or re-visit of our hospital due to infection. Although it is a non-randomized and retrospective study at a single center, our result suggests the usefulness of preventive oral administration of LVFX 250mg before transperineal prostate biopsy.
