真性包茎に対するエストロゲン軟膏の有用性についての検討

Conservative Treatment for True Phimosis with an Estrogen Containing Ointment

　真性包茎患者に対するエストロゲン軟膏（0.1％プレマリン軟膏）の有用性を検討した。対象は5か月〜42歳（中央値3歳）の真性包茎患者29例で，1日1〜2回包皮狭小部に軟膏を塗布するように指導した。2〜27週間（中央値8週間）の治療で，29例中包皮翻転可能になった例は22例（76％），無効5例（17％），脱落2例（7％）であった。頻回の亀頭包皮炎による包皮硬化や指導不足などが無効の原因と考えられた。副作用は軽度乳房腫大1例のみであった。有効22例中，2か月以上追跡できた16例中14例（88％）で治療終了後も効果が持続した。以上より，真性包茎手術適応例ではまずエストロゲン軟膏治療を試みる価値があるものと考えられた。

We evaluated the effectiveness of topical application of an estrogen containing ointment for true phimosis. The oint-ment (not commercially available) was applied once or twice a day to 29 men 5 months old to 42 years old (mean age 3 years) with true phimosis who were referred to our hospital. Of the 29 patients 22(76%) gained foreskin retrac-tibility, 5(17%) remained unchanged and 2(7%) dropped out, during 2 to 27 weeks (median 8 weeks) treatment period. The ointment was not effective in those patients with the stiff foreskin due to recurrent balanoposthitis and with poor understanding on the treatment.
