睾丸固定術後の索状物による精索絞扼

STRANGULATION OF THE SPERMATIC CORD CAUSED BY A FIBROUS CORD

　19歳，男性。左睾丸部痛を主訴に来院，4年前に右睾丸捻転症にて右睾丸固定術を施行。このとき，同時に左睾直丸固定も実施した。左睾丸捻転症の疑いにて，緊急手術を施行。手術の結果，鞘膜内で睾丸白膜から鞘膜に延びている線維性索状物が，精索を絞扼していることが判明した。原因として4年前の左睾丸固定のさいに使用した絹糸が刺激となって，線維性索状物が形成された可能性が考えられた。

A 19-year-old man was seen to our hospital complaining of left scrotal pain. 4 years previously, the patient had undergone right orchiopexy for right testicular torsion and also left orchiopexy on prophylaxis basis. Scrotal exploration showed a thin, fibrous cord running between the tunica albuginea and the tunica vaginalis, which constricted the left spermatic cord. The cord was excised. The previous prophylactic orchiopexy was considered to be responsible for the formation of the cord.
