腹部大動脈瘤による両側尿管閉塞症の2例

BILATERAL URETERIC OBSTRUCTION CAUSED BY ABDOMINAL ANEURYSM; REPORT OF TWO CASES

　腹部大動脈瘤による尿管閉塞症の2例を経験した。CTで腰椎下部に大動脈瘤周囲にリング状に造影される線維化組織を認め，両側尿管は同部で閉塞していた。保存的治療としてダブルJ尿管ステントを留置し，症例1（75歳，女性）では2年10ヵ月良好な腎機能を維持している。症例2（60歳，男性）では2ヵ月後腎機能は改善しなかった。本症の治療における尿管ステント留置の有用性について考察を加えた。

We reported 2 cases of bilateral ureteral obstruction associated with abdominal aneurysm. Bilateral ureter was obstructed at the level of the lower lumbar vertebra (L 4-5). In both cases, CT scans provided valuable information in establishing the diagnosis. It revealed that the ureter was constricted by a soft tissue density mass encompassing the aortic aneurysm. And the mass surrounding the ureter was enhanced by contrast medium.
