体外衝撃波砕石術後に腎盂尿管移行部閉塞をきたした1例

A CASE OF URETEROPELVIC JUNCTION OCCLUSION AFTER EXTRACORPOREAL SHOCK WAVE LITHOTRIPSY

　症例は腹痛と無尿を主訴に受診した41歳の女性．機能的左単腎の腎盂尿管移行部に結石が嵌頓して腎後性腎不全に陥っており，経皮的腎瘻造設術を施行した，腎機能改善の後，体外衝撃波砕石術を施行し結石は破砕され排出したが，1ヵ月後に腎盂尿管移行部の完全閉塞をきたした．体外衝撃波砕石術の合併症としては非常に稀な器質的尿路閉塞の1例を報告した．

A 41-year-old woman was admitted with complaint of abdominal pain and anuria. She had postrenalrenal failure due to a stone at the ureteropelvic junction of the left solitaly kidney, and undergone per-cutanous nephrostomy for the left kidney. After improvement of renal function, extracorporeal shock wavelithotripsy was perfermed and the stone was removed completely. A month later, complete occlusion of theureteropelvic junction was revealed for which surgical plasty was not successful. We report a very rare caseof organic occlusion of urinary tract as a complication of extracorporeal shock wave lithotripsy.
