骨髄移植後にみられたアデノウイルス尿路感染による両側尿管狭窄

A Case of Bilateral Ureteral Stenosis because of Adenovirus Urinary Infection after Bone Marrow Transplantation

　25歳，男性。急性リンパ性白血病にて骨髄移植を受けた。発熱にて内科に入院した。超音波にて両側水腎症を認めたため，当科に紹介された。膀胱鏡では出血性膀胱炎の所見であった。尿中ウイルス検査でadenovirus type 11が同定された。両側W-J尿管カテーテルを留置し，腎不全は軽快した。両側尿管狭窄に対して，左経尿道的尿管切開拡張術を施行し，右経皮的尿管切開拡張術を施行したが，右尿管は完全閉塞となっていたため，右尿管回腸全置換術を施行した。

A 25-year-old male was admitted to our hospital with highfever and renal failure, 7 months after bone marrow trans-plantation, Ultrasonography revealed bilateral hydroneph-rosis. Cystoscopic finding revealed hemorrhagic cystitis.Renal failure was recovered by insertion of W-J ureteralcatheter. Adenovirus type 11 was detected in urine. Bilateralureteral stemosis were pointed out after recovery of hemor-rhagic cystitis. Left transurethral endoscopic ureteral inci-sion and dilatation was done. Right ureteral stenosis waslong and tight, so percutaneous endoscopic surgery wasdone but not successful.
