空腸導管症候群

JEJUNAL CONDUIT SYNDROME

　62歳，女性。止血困難な放射線膀胱炎による膀胱出血に対し尿路変更術を行ったが，回腸が使用できないため空腸導管術を施行したところ，術後空腸導管症候群を発生した。本症候群は空腸における吸収，分泌が他の腸管に比し特異な態度をとるために発生する電解質異常で，病態生理にもとづいて適切に対応すれば比較的容易に管理できるが，尿路変更法に空腸を用いるときには慎重に対処することが必要である。

The patient was a 62-year-old woman who underwent jejunal conduit urinary diversion because of life-threatening hematuria due to radiation cystitis. She complained of nausea, vomiting and general malaise at five postoperative days after ureteral stent were removed. Laboratory data showed hypo-natremia, hypochloremia, hyperkalemia and azotemia. She was treated with intravenous fluids and electrolyte replacement therapy and made an uneventful recovery. The use of jejunal conduit is limited to patients whom the use of other form of diversion is not desirable.
