Kockパウチ術後急性腎不全の1例

ACUTE RENAL FAILURE IN A PATIENTS WITH KOCK POUCH

　Kock禁制回腸膀胱造設11ヵ月後に輸入脚が腸管粘液で閉塞し，急性腎不全を来した稀な1例を報告した。

A 54-year-old man who had undergone construction of Kock's continent reservoir 11 months before developed oliguria and right flank pain. Antegrade pyelography showed dilatation of the afferent valve. This procedure was followed by smooth excretion of urine from the pouch. One month later the patient again developed oliguria. Endoscopy of the pouch confirmed that an abundant amount of mucus was pushed out from the afferent valve by infusion of saline into the nephrostomy. Then, diuresis recovered. A deformity of the afferent nipple valve would have caused accumulation of intestinal mucus leading to postrenal oliguria.
