内分泌非活性傍神経節腫

NON-FUNCTIONING PARAGANGLIOMA

　症例は30歳，女性で右側腹部重圧感の精査にて，右第3腰動脈を主要栄養動脈とし，周囲臓器を圧排する内分泌非活性の後腹膜腫瘍が認められた。腫瘍は全体に被膜におおわれ，組織学的には腫瘍細胞が血管に富んだ間質により区分され，胞巣状構造を呈しており，グリメリウス染色陽性で，良性の副腎外後腹膜腔原発，傍神経節腫と思われたが，傍大動脈領域のリンパ節と思われる小腫瘤にも同様の腫瘍細胞が認められた。

A 30-year-old female patient presented to our hospital with right flank discomfort. She was found to have a hormonally non-functioning retroperitoneal tumor supplied by the third lumbar artery. The tumor was excised and, on gross inspection, it was well encapsulated. Histologically it revealed a typical alveolar pattern and stained positive by Grimelius. Therefore, the diagnosis an extraadrenal paraganglioma was established. Interestingly, despite these benign characteristics of the tumor, a paraaortic lymph node adjacent to the tumor excised operatively had identical histological findings.
