若年者にみられた腎盂尿路上皮癌の1例

Urothelial carcinoma of renal pelvis in a young man

患者は21歳，男性。肉眼的血尿を主訴に来院した。点滴経静脈性尿路造影検査および逆行性腎盂造影検査にて右腎盂腫瘍を疑われた。尿管鏡下の生検で尿路上皮癌の病理診断を得，右腎盂尿路上皮癌cT1N0M0の診断のもと，右腎尿管全摘除術および膀胱部分切除術を行った。文献上，腎盂尿路上皮癌はほとんどが40歳以上に発生し，若年者に発生することはきわめて稀である。

We report a case of a 21-year-old man with 2 weeks' history of macrohematuria found to have a urothelial carcinoma of the renal pelvis. DIP and retrograde pyelography showed an 8 mm irregular filling defect in the right renal pelvis. As histological examination of the specimen taken by ureteroscopy showed grade 2 urothelial carcinoma,right total nephroureterectomy with bladder cuff resection was performed. Review of the literature shows urothelial carcinoma in individuals less than thirty years of age is rare.
