尿路結石の疝痛に対する大腰筋筋溝ブロック法

THE PSOAS COMPARTMENT BLOCK FOR URETERAL COLICKY PAINS

　大腰筋筋溝で尿管の支配神経である陰部大腿神経をブロックすることにより尿管結石の疝痛発作への鎮痛効果を検討した。対象は尿路結石による疝痛発作と診断した症例21名で，男性11名（平均年齢44歳），女性10名（平均年齢47歳）である。疼痛発作時に大腰筋筋溝ブロック法による治療を延べ28回施行した。

　疼痛の消失した患者は23例（82％）で，施行前に比べて疼痛の軽減したのは26例（93％）であった。2例で鎮痛できなかった。この2例は大腰筋筋溝に的確に局所麻酔剤が注入されなかったためと考えられた。全例，血圧低下などの重篤な合併症はなかった。以上の結果より大腰筋筋溝ブロック法は尿管結石の疝痛発作に対する有効な治療法と考えられた。

Using the psoas compartment block, we treated 21 patients with colicky pains due to ureteral stones. Local anesthetic solution 15-20ml was injected from the back, at the level between the 3rd and 4th lumbar transverse processes, into the compartment enclosed by the psoas major and quadratus lumborum muscles and the body of lumbar vertebra. The colicky pains were relieved in 26 patients (93%), although the pains recurred in 8 patients 88 minutes in average after the injection.
