腎尿管疝痛に対する選択的腰部神経根ブロック療法の試み

CLINICAL STUDY OF SELECTIVE LUMBAR NERVE ROOT BLOCK FOR ACUTE RENAL AND URETERAL COLIC

　疝痛発作時の尿路結石症患者15例に対し選択的腰部神経根ブロックを施行した。1％リドカイン液を用い，結石の罹患側の神経根L₁，L₂あるいはL₂，L₃に対し行った。ブロックする神経根レベルは腎の臥位での位置により決め，1つの神経根に対しリドカイン液4mlを用い，1回のブロックで計8mlのリドカイン液を使用した。結果は全例に速効性の除痛効果を認めた（著効80％，有効13.3％，やや有効6.7％，無効0％）。本療法は疝痛発作時の患者に対し，入院を必要とせずに確実に痛みを和らげ，職場復帰を可能とする有用な治療法であると確信した。ブロック手技は慣れれば簡単であり，今後積極的に施行されるべきであると思われた。

A clinical study has been carried out in order to check the effectiveness of selective lumbar nerve root block for acute renal and ureteral colic. The colic was relieved immediately in 93. 3% of 15 patients and spontaneous discharge of calculi was achieved in 46. 7% of 15 patients within 14 days. No serious complications were experienced so far. Thus, these results indicated that the selective lumbar nerve root block was a safe and effective for the treatment of acute renal and ureteral colic owing to calculi.
