長期透析患者の後腹膜腔血腫の2例

TWO CASES OF SPONTANEOUS RETROPERITONEAL HEMATOMA IN LONG-TERM HEMODIALYSIS PATIENTS

　長期透析患者に生じた後腹膜腔血腫の2例を報告した。症例は，45歳と51歳の男性で，突然の腹部痛を主訴として受診，貧血が進行するため，手術を施行した。いずれも多嚢胞化萎縮腎からの出血が原因と考えられ，腎摘出術を行った。長期透析患者の腎に後天性に嚢胞を形成することが知られ，多嚢胞化萎縮腎と呼ぼれている。その合併症として，出血と悪性腫瘍が重要であり，定期的な腎形態のスクリーニングが必要であると思われる。

We described two uremic patients on long-term hemodialysis in whom spontaneous retroperitoneal hematoma developed. The patients, 45-year-old male and 51-year-old male, had been placed on the chronic hemodialysis programs because of chronic renal failure. They visited our hospital with complaints of sudden and progressive left abdominal pain associated with a drop in hematocrit in the absence of obvious blood loss.Left nephrectomy was performed in two cases, and were revealed both massive retroperitoneal hematoma and renal subcapsular hematoma, but no obvious bleeding source was found.
