回腸パウチによる尿禁制型尿路変更術の4例

CLINICAL EXPERIENCE WITH THE ILEALS POUCH FOR A CONTINENT URINARY DIVERSION: A PRELIMINARY REPORT OF 4 CASES

　回腸パウチによる尿禁制型尿路変更術（回腸Sパウチ）を，男性膀胱癌患者4症例に施行した．2例は残存尿道と吻合して，自排尿可能な膀胱置換（再建）術とし，2例は輸出脚を作製して，自己導尿式のユリナリー・パウチとした．術後早期成績としては4症例とも，ほぼ満足すべき結果を得た．回腸SパウチはKockパウチと比較して，手術手技が容易であり，尿禁制型尿路変更術の1つとして，推奨される術式である．

Our preliminary report described the clinical experience with ileal S continent urinary pouches for 4 patients who underwent the radical cystectomy. Two cases were the total replacements of the bladder, and other 2 cases were the continent urinary reservoir (CUR) s. Isolated ileum was arranged in an S shaped fashion to create a urinary pouch and uretero-ileal anastomosis was conducted by Le Duc-Camey procedure. In the early postoperative priode, we obtained the favorable result.
