尿管内腔にポリープ状に浸潤した後腹膜悪性線維性組織球腫

A CASE OF RETROPERITONEAL MALIGNANT FIBROUS HISTIOCYTOMA WITH THE INVASION INTO THE URETERAL LUMEN LIKE POLYP

　51歳，男性。主訴は左下腹部痛。CTなどで左後腹膜腫蕩が疑われた。開腹手術にて，小児頭大の腫瘍が左中部尿管壁内に浸潤し，尿管内腔にポリープ様の隆起を形成していた。病理組織所見にて，悪性線維性組織球腫と診断された。尿管浸潤を認めた後腹膜悪性線維性組織球腫は本邦で6例目である。

A 51-year-old man was admitted to our hospital because of left abdominal pain. The excretory urogram showed left non-visualized ureter and left hydronephrosis. The retrograde pyelography demon-strated the filling defect of left mid-ureter. The abdominal CT scan revealed a large retroperitoneal tumor invading the left ureter. The removal of tumor with partial sigmoidectomy and left nephroureterectomy was done. Histological examination demonstrated malignant fibrous histiocytoma which extraluminaly invaded the ureteral submucosa. The patient passed away 5 months after the operation in spite of the postoperative irradiation therapy.
