治癒的切除のため血行再建を要した後腹膜悪性線維性組織球腫

Retroperitoneal Malignant Fibrous Histiocytoma Resected by Extended Surgical Procedure Combined with Right Common Iliac Artery

　症例は19歳，男性。右腰背部痛を主訴に近医を受診し，右後腹膜腸腰筋前面の腫瘍および右水腎症が認められ、精査，加療目的で当科へ紹介となる。腫瘍部の経皮的生検にて悪性線維性組織球腫と診断された。画像上，腫瘍は右総腸骨動静脈を巻き込んでおり，化学療法施行後に腸腰筋，腸管合併切除，右総腸骨動脈血行再建を伴う広範囲な腫瘍摘除術を施行した。術後2年を経過した現在，再発，転移を認めていない。

A 19-year-old male was admitted to our hospital with the complaint of right flank dull pain. Computed tomography and magnetic resonance image revealed the retroperitoneal solid mass invading the right psoas muscle and causing right hydronephrosis.The needle biopsy was performed under the guide of ultrasonography and the pathological diagnosis was malignant fibrous histiocytoma.Following the chemotherapy with cyclophosphamide, vincristine,adriamycin and dacarbazine, the radical operation was performed with ileo-ileostomy and interposition of right common iliac artery using saphenous vein.
