軟性膀胱鏡の臨床経験

CLINICAL EXPERIENCE WITH FLEXIBLE CYSTOSCOPY

　外来39例，ベッドサイド1例，術中20例，計60例に対して，軟性鏡による膀胱鏡検査および経尿道的処置を行つた。外来における検査は尿道粘膜麻酔下に行つたが，男性患者では検査中の苦痛は非常に少なかつた。体位は原則として仰臥位で行い，そのためベッドサイドでも検査が可能であつた。尿管カテーテル挿入は25例中23例（92％）に成功した。硬性鏡と比べ解像力が劣る，視野が狭い，灌流流量が少ないなどの欠点はあつたが，硬性鏡の内視鏡所見と比較した6例では病変の見落としはなかつた。

Recent advancement in endourology and fiberoptics made flexible cystoscopy possible. Herein we report on our experiences with the flexible cystoscopes. We have attempted flexible cystoscopy on 60 cases and ureteral catheterization on 25 cases with 100% and 92% success respectively. In 6 cases cys-toscopic findings were confirmed by rigid cystoscope. One severely ill patient had bedside cystoscopy under supine posture. All ambulatory male patiens, under topical anesthesia, complained minimal dis-comfort during the procedure.
