75% of bladder trauma are subcutaneous rupture and open trauma are rare. Most of the open trauma reported in the japanese literature are caused by picket stab wound and there are approximately 30 reported cases.

This is a rare case of vesicoileal picket stab wound in a 14 year old middle school student who was stabed in the right inguinal region by an iron rail of the glass door when he was running through the school corridor. The patient recieved a simple suture by an orthopedic surgen soon after trauma, but visited our clinic for peritonitis which occurred after going home and emergency surgery rescued him.
