A CASE OF STAB WOUNDS OF URINARY BLADDER PENETRATING TO COECUM CAUSED BY THE STEEL RAIL OF THE SLIDING DOOR

緒言

　膀胱外傷の75％が皮下破裂で，開放性損傷は稀である。本邦では，開放性損傷のうち膀胱刺抗創が大部分を占め過去30余例の報告がある^1〜4）。　本症の受傷部は外見上小さく，かつ出血もわずかであるが，創傷は意外に深部に達して重要臓器の損傷がかくされていることがあるので，診断には十分注意を要する。

　受傷後整形外科医で簡単な縫合処置を受けていつたん帰宅後腹膜炎を起こして来院し，ただちに手術を行なつて救命し得た腹腔内臓器損傷を伴つた本症の1例を経験したので報告する。

75% of bladder trauma are subcutaneous rupture and open trauma are rare. Most of the open trauma reported in the japanese literature are caused by picket stab wound and there are approximately 30 reported cases.

This is a rare case of vesicoileal picket stab wound in a 14 year old middle school student who was stabed in the right inguinal region by an iron rail of the glass door when he was running through the school corridor. The patient recieved a simple suture by an orthopedic surgen soon after trauma, but visited our clinic for peritonitis which occurred after going home and emergency surgery rescued him.
