A 61-year-old female with right ureteral stone went into shock 36 hours after admission. Cepha-lothin was injected intravenously for suspected bacterial shock but skin necrosis developed at the 3 injected site after sever pain. Skin transplantation was performed after recovery from bacteriemia. E. coli was isolated by culture of patients blood.

Necrosis of the skin can occur only from bacterial shock secondary to damage of the endothel cell of the blood vessel. Phlebitis secondary to intravenous administration of cephalothin has been reported.
