根治的膀胱全摘出術の際に広範な腹膜欠損を生じたため死腔を大網を用いて被覆した12例と死腔を腹膜のみで被覆した対照の38例との間で,術後合併症について比較した。術後合併症の発生頻度に有意差はなかつたが,大網利用群に生じた死腔感染は容易に治癒し,対照群の死腔感染は難治陸であつた。この結果から,膀胱摘出後の死腔を大綱で被覆することは,難治性死腔感染防止に有効と考えられる。
Twelve patients underwent a radical cystectomy where the dead space was covered with a pedicled omentum because of a large peritoneal defect. The results were compared with those of 38 patients in whom the dead space was covered with the peritoneum alone. Although no significant difference in the incidence of postoperative complications was found between the two groups, dead space infection in the omental group healed readily, while that in the control group required curettage or longtime management. These results indicate that the use of pedicled omentum is effective in preventing incurable dead space infection.
