嚢胞形態を示した腎盂腫瘍の1例

RENAL PELVIC TUMOR PRESENTING A CYSTIC APPEARANCE: A CASE REPORT

　血尿を主訴とする78歳の男性が，DIP,CT，血管撮影で，左腎上部に直径約16cmの不整な壁を持つ嚢胞腫瘤を示した。根治的腎摘除術を施行したが，移行上皮で被われた嚢胞内壁の一部に移行上皮癌が存在し，嚢胞に接する腎杯にも移行上皮癌が認められた。移行上皮癌が腎杯狭部に発生し嚢胞状水腎杯を形成したと考えた。

A 78-year-old man, presenting with gross hematuria, had a cystic mass of 16 cm in diameter in the upper pole of the left kidney. Radical nephrectomy was performed. Transitional cell carcinoma was found in the cyst wall which was covered with transitional epithelium and also in the calyx adjacent to the cyst. We considered that tumor developed at calyceal infundibulum, leading to a cyst-like hydrocalyx.
