Dimple signを示した腎盂腫瘍の1例

A CASE OF RENAL PELVIC TUMOR DEMONSTRATING "THE DIMPLE SIGN"

緒言

　腎盂腫瘍に対する腎血管造影の診断的価値は，腎細胞癌に対するそれよりも低いと言われている。しかし，多くの所見が報告され，現在では補助的診断法としての価値は認められているようである。1974年Baltaxe^1）らにより，乏血管性腎細胞癌の血管造影でdimple signなる所見が提唱され，多田ら^2）により腎盂腫瘍においても認められたと報告された。今回，浸潤性の腎盂腫瘍で，dimple signと思われる所見を認めたので報告する。

A 55-year-old male was admitted for painless hematuria. A right selective renal angiogram did not reveal the lower segmental artery and demonstrated a retraction of the cortex, "dimple sign" termed by Baltaxe. Retrograde pyelogram showed a fiilling defect of lower major calyx. A right nephrourete-rectomy with bladder cuff resection was performed. The gross examination of the kidney demonstrated atrophy of the cortex adjacent to the tumor. The histopathologic finding was invasive transitional cell carcinoma.
